Custom Graphics Worksheet
Start the process of ordering your custom artwork here. Use this form to give instructions on creating your custom artwork, what sample shirt(s) you'd like to receive, where to print the art, your event date, other matching products you'd like to order and when you need them delivered, and payment method for your deposit. Note that no products will be billed until after you have seen and approved proofs of the art and layouts. If you are ordering any other products that need typesetting, such as posters, tickets, program covers, certificates, etc., also fill out and send along the Data Collection Sheet.

T-Shirt Sign-Up Sheet 
You'll get a copy of this Sign-Up sheet in your Sample Pack that comes with your Free Proof Sample Shirt, but it is to download and print  in case you run out. Use one form for each style or color of shirt. (For best sales results, and for a more unified look, we recommend no more than 3 styles and/or colors of shirt.)

Plush Stuffed Animal Sign Up Sheet
Kids love stuffed animals and we have a whole menagie to help make recital even more fun and memorable! Particularly if you have a theme related to animals, the jungle, the circus, Wizard of Oz, Peter and the Wolf, Lion King, fairy tales and more, our Stuffed Plush Animals can complement your theme and will be a big hit with the kids. And for light-shirt process, these count towards minimums and price breaks. With your Free Proof Sample Shirt, you'll receive a custom full-color sales poster showing your art on stuffed animals, along with this sign-up sheet. Use this form to take orders. All animals come with a mini-t-shirt printed with your matching art. Or in the case of our Wolf and Zebra, mini bandanas.
